How encapsulated HTTP heads carry a protocol upgrade: an HTTP/1.1 Upgrade, or the
:protocol of an HTTP/2 or HTTP/3 Extended CONNECT, which is encapsulated as the GET
upgrade it stands for, and its 2xx acceptance as a 101 (RFC 8441 §5).
RFC 3507 §4.4.2 omits hop-by-hop fields from encapsulated heads; established ICAP clients
keep Upgrade, so a service tells an upgrade from a plain GET. Connection is never encapsulated. Without
Upgrade an acceptance stays the 2xx it was, as a 101 must name its protocol.
